What is All-Laser LASIK?

Bladeless or All-Laser LASIK is a type of laser eye surgery that doesn’t use blades but two different kinds of lasers to correct vision problems like nearsightedness, farsightedness, and astigmatism.

During the procedure, LASIK corrects the patient’s vision by altering the curvature of the cornea – the transparent outer layer of tissue that covers your eye.

On the main LASIK page, we have described the 6 steps of the LASIK procedure in some detail. On this page, we would only like to focus on three distinct stages of All-Laser LASIK:

The All-Laser LASIK Procedure

The creation of a flap in the cornea is done using a special type of laser called a femtosecond laser rather than a bladed instrument called a microkeratome. This adds an extra layer of safety to the procedure.

The flap is lifted so that the surgeon can apply a second type of laser to the underlying surface and remodel the cornea to correct your specific prescription. The laser used for treatment is therefore different than the one that creates the flap. The second laser allows light to perfectly focus in your eyes and form a sharp image. The type of laser treatment used in this stage is a distinct consideration from the one that forms the flap and is still very important to the visual quality and sharpness. Many centers that use the bladeless methods may use an inferior laser for the treatment part to cut costs. This may result in an inferior visual outcome. What is Corneal Cross Linking

Technical LASIK Steps

Step 1

The creation of flap in the cornea is done by using a special type of laser called a FEMTOSECOND LASER, rather than a bladed instrument called a microkeratome. This adds an extra layer of safety to the procedure.

Step 2

The flap is lifted so that the surgeon can apply a different laser to the underlying surface and remodel the cornea to correct your specific prescription. The second laser allows light to perfectly focus in your eyes and form a sharp image. Make sure you are aware of which type of laser correction you are getting. Bladeless LASIK by itself is not specific enough therefore this may result in an inferior visual outcome. The treatment here could be either prescription-based or topography-guided Lasik/Contoura®

The flap is repositioned and adheres to the underlying tissue. This permanently corrects your vision

Creating the Flap

In traditional LASIK, the flap is created by using a device called a microkeratome, which contains a blade.

In All-Laser LASIK, at the Cohen Eye institute, we perform 100% of our LASIK procedures using a femtosecond. This is known as All-Laser LASIK or Bladeless LASIK.

In the past, when flaps were created manually using a microkeratome blade, there were more frequent flap complications which could prevent the surgery from being completed or even affect the quality of vision.

Using a femtosecond laser, Dr. Cohen is able to create a precise flap with greater accuracy and safety.

All-Laser LASIK is the safest form of LASIK surgery currently available.

Reshaping the Cornea

Once the flap is created, a different type of laser called an excimer laser is used to reshape the cornea.

There are different types of excimer lasers on the market and at Cohen Eye Institute, we only use the best laser that is available – the Alcon Allegretto 500.

This device offers the sharpest vision compared to other older models (VISX, NIDEK, etc.). It is also the only platform that allows a type of treatment customization called Topography-guided LASIK or Contoura®.

FDA studies show that the Contoura® (this) has been able to provide visual acuities that are superior to 20/20 vision (the “normal” vision that a person can see from 20 feet away).
